Volunteer WAIVER

This release has been executed October 2023 by the below named individual (or his/her Parent or

Legal Guardian, if a minor) herein referred to as Releaser, to the Dead Time Dreams Haunted

Attractions of San Jose, California, herein referred to as Release.

Releaser, being of lawful age, and in consideration of being permitted to participate in the

activities of the Dead Time Dreams Haunted Attractions, do for myself, my heirs, executor,

administrators and assigns, hereby release and forever discharge the Dead Time Dreams

Haunted Attractions, its Directors, Officers, and/or members of and from any and every claim,

demand, action or right of action, or whatever kind or nature, known or unknown, either in law

or equity, arising from or by reason of any bodily injury or personal injuries known or unknown,

death, illness or property damage resulting or to result from any accident which may occur as a

result of participation in the activities of the Dead Time Dreams Haunted Attractions, whether by

negligence or not.

Releaser agrees to indemnify the Release from any loss, liability, damage or cost they may

incur due to the presence of Releaser in or upon the premises where the Dead Time Dreams

Haunted Attractions activities take place, whether caused by the negligence of the Releaser or

otherwise.

Releaser assumes full responsibility for the risk of bodily injury, death, illness or property damage

due to the negligence of Releaser or otherwise while in, around, or upon the premises where

the Dead Time Dreams Haunted Attractions activities take place and ANY QUESTION OR

Releaser expressly agrees that this release, waiver and indemnification agreement is intended to be

as broad and inclusive as permitted by the laws of the State of California, and that if any

portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue

in full force and effect.

All participants must be at least 18 years of age OR have signed consent from their

parent/legal guardian. ALL participants are responsible for arranging transportation to and

from Dead Time Dreams Haunted Attractions events; the Dead Time Dreams Haunted

Attractions WILL NOT be responsible for, nor provide, transportation for minor participants.

I HAVE READ AND UNDERSTAND THE TERMS OF THIS AGREEMENT AND I

HAVE EXECUTED THE SAME ON THE ABOVE WRITTEN DATE.
